Fairview Health Services in Rosemount, MN, seeks a Family Medicine Physician to join the M Health Fairview Clinic – Rosemount. You’ll join a 11-provider primary care team delivering high-quality care with top-decile patient satisfaction and a balanced schedule.

This full-time role offers 34 patient contact hours per week with 18–20 patients per day, options for virtual care, and a 1:6 call with nurse triage. The clinic uses EPIC EMR within a Pioneer ACO network.

Qualifications

• Graduate of an accredited ACGME/AOA Family Medicine residency program.
• BC/BE with ABMS or AOA.
• Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice; DEA certification.

Job Overview

Fairview is seeking a Family Medicine Physician to join our primary care practice at M Health Fairview Clinic – Rosemount. Join our very high performing clinic with top-decile clinical quality and patient satisfaction scores. Practice Details:

• 11 provider primary care practice which includes Family Medicine, Internal Medicine, Pediatrics, and Nurse Practitioners and Physician Assistants
• Specialists in Behavioral Health Consultant, Certified Diabetes Educator, anticoagulation, medication therapy management, and an onsite pharmacy.
• 18-20 patients per day.
• Virtual care offered via video, telephone, and through EMR.
• 34 patient contact hours per week for full time - flexible schedule.
• Call Frequency: 1:6 low intensity - phone consultation only with first tier nurse triage
• As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
• Fully integrated Electronic Medical Record (EPIC).

Qualifications:
