Practice Details

Sanford Clinic Canby is seeking two physicians who are BE/BC Family Medicine to join a physician-led practice serving the communities of Canby, MN and Clear Lake SD.

• Primary location will be Canby, MN with responsibilities in Clear Lake, SD
• Sanford Clinic Canby’s current medical staff consists of 2-FM, 1-IM, 1-General Surgeon and 3-Advanced Practice Providers
• The ED is a Level 4 trauma center – ED call is 1:6 / 8 weekends per year
• Average number of visits per year is 1,000
• Sanford Clinic Clear Lake’s current medical staff consists of 1 MD and 1 APP
• Sanford Clinic Clear Lake is attached to a 25-Bed critical access hospital

Canby, MN is located 100 miles from Sioux Falls, SD and 150 miles from Minneapolis, MN. Canby offers a wonderful quality of life with excellent schools, hospital, nursing home, wellness center, and an abundance of recreational and leisure activities.

Job Description

Family Medicine Core Privileges includes the ability to perform history and physical examination, admit (if applicable to the setting), evaluate, diagnose, treat and provide consultation to patients of all ages with a wide variety of illness, diseases, injuries, mental health conditions and functional disorders of circulatory, respiratory, endocrine, metabolic, musculoskeletal, hematopoietic, gastroenteric, integumentary, nervous, female reproductive, and genitourinary systems. This includes the pre-operative care of surgical patients and post-operative medical care of surgical patients and return to work issues.

Qualifications

M.D. or D.O. degree.

Experience can be demonstrated in one of the following ways and should be reflective of the privileges being requested: An applicant who has just completed residency shall provide their residency procedure log and a list of completed rotations, as applicable. OR An applicant who is not applying directly out of a residency shall provide a quality profile from hospital(s) where he/she currently has privileges showing his/her clinical activity for the last 12 months, including numbers of procedures performed morbidity, mortality, infection rates, and other complications. Applicant must be able to demonstrate active involvement in the clinical aspects of Family Medicine by providing case log that includes 25 vaginal deliveries, 50 adult medical patients, and 10 inpatient care of children under the age of 18 (excluding normal newborn), as applicable to privileges being requested, in the last 12 months.

Successful completion of a Family Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or he Royal College of Physician and Surgeons of Canada. Training at residency programs accredited by organizations other than the above, may be considered via the waiver of eligibility process as defined in the Credentialing Policy. Certification in the primary area of practice by the appropriate specialty/ subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA), as applicable. Those applicants who are not board certified at the time of application but who have completed their residency training within the last five years shall be eligible, however must achieve board certification within five years from the date of completion of their residency training unless the appropriate governing Board waives such requirement. (Refer to each entity’s policy for grandfather exceptions)

Compensation

Sanford Health offers a nationally competitive compensation plan with an additional physician benefits package including a health, dental and vision insurance, 401K plan, short-term and long-term disability, life insurance, CME allowance, allowed time away, malpractice insurance and tail coverage, and a relocation allowance.
Annual compensation floor range (based on 1.0 FTE): $265,175 - $343,168 plus estimated potential to exceed compensation floor from personally performed wRVUs.
Additional incentives may be offered. Actual compensation offers may vary and are determined based on multiple factors including education, training, certification, experience, scope of practice and actual job requirements.
