Job summary

ChenMed LLC is seeking a Nephrology Extender to enhance care for CKD and ESRD patients in the United States. The successful candidate will coordinate quality care between primary care physicians, community nephrologists, and dialysis clinics, reporting directly to the National Director of Nephrology.

Applicants should be Licensed Physician Assistants or Nurse Practitioners with extensive nephrology experience, capable of analyzing gaps in care and improving patient outcomes.

Qualifications

• Preferred certification in nephrology nursing or CAQ.
• Strong experience in care coordination and patient education.

Job description

Job Overview

The Nephrology Extender coordinates quality care for CKD and ESRD patients between primary care physicians, community nephrologists, and dialysis clinics, reporting to the National Director of Nephrology.

Responsibilities

• Analyze care gaps and share recommendations with PCPs and dialysis clinics to reduce crisis entry into dialysis.
• Coordinate care among the ChenMed Nephrology team, PCPs, dialysis units, and vascular or inpatient facilities.
• Review patient charts, provide assessments, and recommend care plans.
• Enhance home dialysis utilization, emphasizing de‑novo starts.
• Collaborate with the transitional care team to reduce ESRD hospitalizations and length of stay.
• Provide virtual education to patients on CKD and dialysis options.
• Develop trusting relationships with internal and external stakeholders to identify opportunities and devise remediation strategies.
• Support and complete ad‑hoc initiatives including research and care advancement proposals.
• Travel up to 75% as required.

Scope of Work and KPIs

• Late‑stage CKD (near dialysis)
  • Analyze gaps in care and recommend action to Dr. Kheda/PCP.
  • Manage nephrology and transplant referrals.
  • Review charts and assess candidacy for home dialysis.
  • Assist in developing CKD patient education materials.
  • Provide virtual education on CKD and dialysis options.
  • Coordinate with community nephrologist to avoid “crashing into dialysis.”
  • Increase de‑novo home dialysis starts.
  • Reduce crisis entry into dialysis.
• ESRD on hemodialysis
  • Analyze gaps and recommend actions to Dr. Kheda/PCP/dialysis clinics.
  • Conduct virtual assessments for transition to home dialysis.
  • Coordinate with TCM team to reduce hospitalizations and length of stay.
  • Facilitate coordination between ChenMed, dialysis unit, vascular, and inpatient care.
  • Increase proportion of patients on home dialysis.
  • Decrease hospitalization rate and length of stay.

Qualifications

• Licensed Physician Assistant or Nurse Practitioner with 5+ years of nephrology experience.
• Nephrology CAQ or Certified Nephrology Nurse (CNN) preferred.
• Experience in care coordination and coaching.
• Excellent verbal and written communication skills.
• Intermediate proficiency with Microsoft Office (Excel, Word).
• Experience with Medicare and/or Medicare Advantage preferred.
• Experience in value‑based care (e.g., ESCO) or risk‑model based healthcare a plus.

Pay Range

$111,140 – $158,771 (annual salary). Final compensation may vary based on experience, education, location, and other factors; bonuses or commissions may be applicable.
