1 Care Partners in Richmond, VA is seeking an experienced Physician Assistant (PA) to provide high-quality care in a Skilled Nursing Facility (SNF) serving geriatric residents.

In this full-time role you will collaborate with physicians, nursing staff, and a multidisciplinary team to assess, diagnose, and manage residents' health, develop treatment plans, and ensure regulatory-compliant documentation and continuity of care.

Qualifications

• Active Virginia PA license required.
• Experience in Skilled Nursing Facility (SNF) or geriatric care preferred.
• Strong clinical assessment and decision-making skills.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Diagnose and manage acute and chronic conditions in coordination with physicians.
• Educate patients, families, and facility staff on disease management and preventive care.
• Maintain accurate and timely documentation in compliance with regulatory guidelines.
