Job summary

Brattleboro Memorial Hospital in Brattleboro, VT is seeking a Physician Assistant for Post Acute Care. Work Monday–Friday 8am–5pm with 1:2 call schedule, in a post-acute care setting focusing on transition of patients from acute care to rehab, SNF, or home-based care.

The role requires PA certification, VT PA-C licensure, and 2 years of PA experience. Competent in care coordination, patient education, and regulatory compliance with a patient-centered approach.

Qualifications

• Certification/degree from an accredited Physician Assistant program.
• Two years PA experience in a primary care or relevant specialty setting preferred.
• Current licensure as a Vermont PA-C and national PA certification recognized by the Vermont State Board of Medical Practice.

Responsibilities

• Provide comprehensive evaluation, treatment, and care management for patients transitioning from acute care to post-acute settings.
• Collaborate with interdisciplinary teams to develop individualized care plans and promote recovery and independence.
• Document treatment plans and outcomes, educate patients and families, and ensure regulatory, quality, and safety standards are met.
