We are looking for creative thinkers and self-starters who can help grow and shape our program to meet the evolving needs of our patients and their families. *Your Role*** As an *Advanced Practice Provider* in our Palliative Care Team, you will: • Conduct comprehensive physical and psychosocial assessments of patients. • Create and implement evidence-based, patient-centered care plans. • Provide guidance and education to patients and their families on disease management, symptom control, and navigating care transitions. • Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and chaplains, to ensure seamless, compassionate care. • Offer expertise in managing symptoms such as pain, breathlessness, nausea, fatigue, and more. • Serve as a key resource for patients and referring providers, ensuring continuity and quality of care across settings. This position primarily involves *Monday-Friday, 8 AM-5 PM clinic care*, giving you the opportunity to provide deeply impactful care while maintaining work-life balance. *About You*** We are looking for a *team player* with excellent communication skills and a passion for delivering patient-focused care.
