Twin Cities Physicians in the Greater Twin Cities Metro Area is seeking a full-time Physician Assistant to provide on-site rounding in senior living communities. You will manage chronic conditions, coordinate care, and help patients navigate complex healthcare needs with strong clinical judgment.

This role emphasizes geriatrics and preventative care, offering a full benefits package, mileage reimbursement, PTO, CME allowance, malpractice coverage, and a compensation package ranging from $120,000

Qualifications

• Active Minnesota PA-C license in good standing.
• Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, or post-acute care preferred.
• Strong clinical judgment and patient-centered approach with good communication.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide on-site rounding care in assisted living, memory care, and long-term care communities.
• Perform physical examinations, chronic disease management, preventative care, and follow-ups.
• Develop and adjust individualized care plans based on patient needs.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Communicate treatment plans and patient progress with families and care teams.
• Support continuity of care and help reduce unnecessary ER visits and hospitalizations.
• Complete documentation, medication management, and lab coordination.
