What You’ll Do:

️ Provide in-home or on-site care at private residences, senior living communities, and long-term care settings

️ Manage chronic conditions, preventive care, and mental health needs for an aging population

️ Collaborate with interdisciplinary teams to deliver whole-person, senior-focused care

️ Build trusted relationships through empathy, consistency, and patient education
