Ideal candidates will have advanced training in geriatrics, an APRN License, and experience in primary care. Qualifications

  • State of Florida Advanced Practice Registered Nurse License and ACLS Certification required.
  • At least 1 year experience as an APRN and preferred geriatrics experience.
  • 5 years experience as a RN and 1 year working with the frail or elderly preferred.

Responsibilities

  • Oversee and manage high-risk geriatric participants.
  • Conduct assessments and perform physical assessments.
  • Participate in the development of plans of care with the interdisciplinary team.
  • Conduct rounds in Eldercare contracted nursing homes, hospitals, and other care sites.
  • Perform home visits as needed for medical interventions.
  • Contribute to care-planning with the interdisciplinary team and develop plans of care.
  • Coordinate 24-hour care delivery and participate in team meetings.
  • Integrate the primary care plan into the Eldercare PACE plan of care.
  • Assist with discharge planning and ensure safe transitions between care levels.
  • Provide preventive care, immunizations, screenings, and monitoring per Eldercare protocols.
  • Prudently prescribe medications and therapies for participants.
  • Demonstrate advanced knowledge of diagnostic and therapeutic procedures for at-risk elders.
  • Interview patients and document comprehensive medical history and physicals within PACE policies.

Position Responsibilities:
  • Under the direction of the physician he/she will be assigned a panel of high-risk geriatric participants to oversee and medically manage.
  • In collaboration with a physician, conduct initial, semi-annual and unscheduled assessments and reassessments per policy and regulations and perform additional physical assessments of participants as needed.
  • Assist in the daily oversight of the Eldercare Clinic. Including clinic nurses, home care and UR nurse, medical assistants and other technicians..
  • Perform rounds in Eldercare contracted nursing homes, hospitals and other care sites as needed.
  • Conduct home visits as needed for medical interventions.
  • Participate in the development and implementation of participant plans of care as part of and in collaboration with the interdisciplinary team.
  • Function as a member of the interdisciplinary team. Maintains regular attendance at, and participate in interdisciplinary team meetings; communicate participant changes, collaborate on care planning decisions and coordination of 24 hour care delivery.
  • Integrate the primary care treatment plan into the overall plan of care developed in collaboration with IDT (Interdisciplinary Team)
  • Interact with team members to meet emergent and acute needs of participants..
  • Reviews all hospital, ED or other acute settings admission information and assists the IDT team with the discharge plan and services needed to ensure a safe transition from one level of care to another.
  • Provide preventive health care and maintenance for participants, including immunizations, screenings and monitoring of pertinent indicators. Follow the Eldercare’s clinical protocols
  • Prudently prescribe medications, therapies and other treatments for participants.
  • Demonstrates advance knowledge of the general diagnostic and therapeutic procedures for the at risk participants of the Eldercare PACE program
  • Interviews patients and completes a full comprehensive medical history and physical examination within PACE regulations and Eldercare policies.
Qualifications:
  • State of Florida Advanced Practice Registered Nurse License; ACLS Certification
  • Advanced Practice Registered Nurse, Geriatric Certification preferred. Master of Science in Nursing
  • At least 1 years experience as an APRN (preferable), primary care or other medical specialty preferable geriatrics experience preferred, 5 years experience as a RN and 1 year working with the frail or elderly
