Job summary

Mount Sinai Medical Center in Florida is seeking a Certified Nursing Assistant to join Eldercare. You will assist with daily living activities, monitor vital signs, support meals, and help maintain a clean, safe care environment.

Work as part of an interdisciplinary team, document observations, accompany participants to appointments, and contribute to fall prevention and crisis de-escalation. Florida CNA certification, CPR/First Aid, HS diploma preferred, and at least 1 year of elder care

Qualifications

• Florida Nursing Assistant Certification is required.
• CPR and First Aid Certification required (First Aid may be obtained within 90 days).
• 1 year of experience working with frail or elderly population preferred.

Responsibilities

• Participate as a member of the interdisciplinary team as needed.
• Carry out care plan activities within the CNA scope of practice.
• Assist participants with activities of daily living (bathing, grooming, feeding, toileting) as assigned.
• Create and participate in activities per daily calendar and with the Activity/Recreational Coordinator.
• Maintain clean and orderly care center rooms and service areas.
• Accompany enrollees to medical appointments as requested.
• Assist transportation staff with loading/unloading of participants.

Job Description Summary:

Position Responsibilities

• Participate as a member of the interdisciplinary team on an as needed basis.
• Carry out plan of care activities within the CNA scope of practice as directed.
• Assist and/or supervise participant's with activities of daily living in the center or when asked, in the participant’s home to include (i.e., bathing, skin care, dressing, grooming, Foley care, decubitus preventive care, hair care, mouth care, eating, hand and food nail care, and toileting) as assigned per the care plan under the direction of the supervisor.
• Create and participate in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Activity/ Recreational Coordinator
• Throughout the day, assist in maintaining clean and orderly day care center rooms, serving area, personal care areas and other areas of the center as directed.
• Accompany enrollee to medical and other appointments as requested.
• Assist transportation staff with the loading and unloading of participants from vehicles.
• Serve meals and assist participants as needed with feeding & other needs as necessary. Reports participant food & fluid intake as needed into medical record and communicates any issues to the Activity Coordinator or Registered Dietician.
• Performs skills/ services according to accepted procedures. These include taking vital signs, weight, height, collecting specimen, enema administration, performing Sure Step blood sugar test, measuring intake & output and assisting with participant group activities.
• Reports any changes observed in the condition or behavior of participant to appropriate Eldercare personnel.
• Maintains availability to assist during crisis or emergency situations.
• Responds to participant call light/ request in a timely manner and follows through on participant need/request.
• Accurately documents participant observation following Eldercare protocol.
• Actively participates in Eldercare's Fall Prevention program.
• Adheres to the principles of non-violent crisis intervention when dealing with aggressive participants. Assists in the de-escalation process.
• Maintains therapeutic communication with participant. Gives re-direction and/ or reality re-orientation or validation as needed.
• Attends departmental meetings and in-service programs.
• Know and adhere to the philosophy and goals of Eldercare.
• Act only within the scope of authority to practice.
• Participate in and support Quality Initiatives

Qualifications

• License/Registration/Certification
  • State of Florida Nursing Assistant Certification. CPR and First Aid Certification required. First Aid may be completed in first 90 days.
