Provide the full scope of services within the provider's field of training, including diagnosis, treatment, care coordination, preventive care, and electronic health record (EHR) maintenance, following guidelines in quality management, risk management, infection control, customer service, and safety programs for Access Community Health Network.

Clinical and Operational Duties

  • Obtain health histories and perform physical examinations
  • Diagnose, treat, counsel, and coordinate care for patients
  • Review and update problem lists and care plans at each visit
  • Complete documentation and progress notes within 24 hours of visits
  • Respond to on-call responsibilities within 20 minutes as scheduled
  • Prescribe medications per Illinois law and professional guidelines
  • Adhere to quality standards and clinical guidelines established by ACCESS
  • Meet Meaningful Use standards using certified EHR technology to improve care quality, safety, and efficiency
  • Manage EHR In-Basket messages and respond within 72 hours
  • Ensure activities meet reimbursement guidelines
  • Provide culturally competent care and involve families in care plans
  • Address patient and family questions and concerns
  • Encourage patient self-management and conduct education activities
  • Use appropriate health education materials and document instructions given
  • Coordinate with care team and case management for comprehensive care
  • Transfer patients with higher acuity appropriately
  • Meet severity of illness admission criteria

PCMH Requirements

  • Coordinate with health center staff to meet patient needs
  • Participate in huddles and meet productivity standards
  • Ensure timely and appropriate patient care involving all staff
  • Carry out treatment and educational tasks according to policies

Provider Development

  • Participate in staff orientation and training
  • Ensure staff meet certification requirements for education and medication administration
  • Participate in proficiency testing
  • Serve as a preceptor for training programs
  • Follow HRSA and FTCA regulations
  • Perform other duties as assigned

Requirements/Preferences

  • Current Illinois PA license and CME compliance
  • Board certification required
  • Obtain and maintain collaborative agreement and DEA license
  • Two years outpatient primary care experience preferred
  • Knowledge of quality assurance, safety, infection control rules
  • EPIC EHR competency preferred
  • Cultural competency and interpersonal skills necessary
  • Strong organizational skills required

Working Conditions/Equipment

  • Ability to focus amid interruptions

ACCESS is a network of Federally Qualified Health Centers serving community-based health care. Vaccination against communicable diseases may be required unless exemptions are granted. Compensation varies based on experience, education, market conditions, and funding.
