Chestnut Health Company in Payson, AZ, seeks a Physician Assistant to join our physiatry team within Physical Medicine and Rehabilitation. This role emphasizes collaborative care and patient-centered outcomes in a multidisciplinary setting.

Qualifications

Must be a licensed PA in the state of employment.
Current Controlled Substance License and DEA registration required on start date.
Interest in Physiatry/Physical Medicine and Rehabilitation.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans.
Conduct patient assessments, including histories, exams, and diagnostics to set rehab goals.
Monitor patient progress and adjust plans to optimize functional outcomes and independence.
Participate in multidisciplinary meetings and educational activities to enhance skills.
Maintain accurate documentation of evaluations, plans, progress notes, and discharges in EMR.
Educate patients and families about rehabilitation process, outcomes, and post-discharge care.

Essential Duties & Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
