You will evaluate wounds, develop and revise treatment plans, and perform procedures within your scope while traveling to skilled nursing facilities and other locations. The role requires strong clinical judgment, excellent documentation, and the ability to work independently in the field, with coordination with physicians and the interdisciplinary care

Qualifications

• Graduation from an accredited Physician Assistant program.
• Current and unrestricted PA license in the state(s) where services are provided.
• Current NCCPA certification, if applicable.
• Current DEA registration and other prescribing credentials when required.
• Current BLS/CPR certification.

Responsibilities

• Perform comprehensive wound evaluations and develop individualized treatment plans.
• Manage acute, chronic, and complex wounds including various ulcers and injuries.
• Order and interpret diagnostic tests and imaging as appropriate for wound management.
• Provide education to patients and care teams on wound care, prevention, and nutrition.
• Maintain accurate documentation and ensure compliance with HIPAA and state/federal requirements.
• Travel to assigned facilities and collaborate with interprofessional teams.

Physician Assistant – Mobile Wound CarePosition SummaryThe Physician Assistant (PA) provides high-quality, patient-centered wound care services in a mobile clinical setting, including skilled nursing facilities, long-term care facilities, assisted living communities, and other patient care locations served by the company.Working collaboratively with physicians, nurse practitioners, nursing staff, facility leadership, and other members of the interdisciplinary care team, the PA evaluates and manages acute, chronic, and complex wounds while developing individualized treatment plans designed to promote healing, reduce complications, and improve patient outcomes.This position requires strong clinical judgment, excellent documentation skills, the ability to work independently in the field, and a commitment to providing compassionate and evidence-based wound care.Essential Duties and ResponsibilitiesPerform comprehensive wound evaluations, including wound measurements, staging/classification, assessment of tissue characteristics, drainage, surrounding skin, circulation, infection risk, and other factors affecting healing.Evaluate and manage acute, chronic, non-healing, and complex wounds, including pressure injuries, diabetic ulcers, venous and arterial ulcers, surgical wounds, traumatic wounds, skin tears, and other wound conditions within the PA's scope of practice.Develop, implement, and update individualized wound care treatment plans based on clinical findings and patient needs.Perform appropriate wound care procedures within scope of practice, training, credentialing, and applicable state and federal requirements, including debridement when clinically indicated and authorized.Order and interpret appropriate diagnostic testing, laboratory studies, imaging, cultures, vascular studies, and other evaluations related to wound management when clinically appropriate.Prescribe medications, wound care products, dressings, compression therapy, offloading interventions, and other treatments within applicable scope-of-practice requirements.Identify signs of infection, vascular compromise, deterioration, or other complications and initiate appropriate treatment, referrals, or escalation of care.Assess factors that may interfere with wound healing, including nutrition, mobility, pressure, circulation, diabetes management, medications, and underlying medical conditions.Coordinate referrals to specialists or higher levels of care when clinically necessary.Provide patient, family, caregiver, and facility staff education regarding wound care, prevention, pressure relief, positioning, skin protection, nutrition, treatment compliance, and other factors affecting wound healing.Collaborate closely with facility nursing staff and other healthcare professionals to promote continuity of care and implementation of treatment recommendations.Communicate significant changes in patient condition promptly to the appropriate physician, facility staff, family, or other members of the care team.Complete accurate, thorough, and timely clinical documentation for each patient encounter in accordance with company policies, payer requirements, and applicable regulatory standards.Ensure documentation supports medical necessity, services performed, treatment decisions, procedures, diagnoses, and the patient's ongoing plan of care.Maintain compliance with HIPAA, OSHA, Medicare/Medicaid requirements, company policies, clinical protocols, and all applicable federal and state regulations.Participate in quality improvement, clinical education, case review, and other initiatives designed to improve wound healing outcomes and patient care.Maintain required professional licenses, certifications, continuing education, credentialing, payer enrollment, and other qualifications necessary for the position.Travel to assigned facilities and patient care locations according to the established clinical schedule.Maintain professional relationships with facility administrators, nursing teams, patients, families, referral partners, and other healthcare professionals.Represent the company professionally and support positive, collaborative relationships with the facilities and communities we serve.Perform other clinical and administrative duties reasonably related to the position as assigned.Equal Employment OpportunityThe company is an equal opportunity employer and considers qualified applicants without regard to race, color, religion, sex, national origin, age, disability, veteran status, or any other characteristic protected by applicable law.QualificationsGraduation from an accredited Physician Assistant program.Current and unrestricted Physician Assistant license in the state(s) where services are provided.Current NCCPA certification, as applicable.Current DEA registration and other prescribing credentials when required for assigned duties.Current BLS/CPR certification.Wound care experience preferred;
