Job summary

Port Jefferson Medicine is seeking a Physician Assistant to join a private practice known for compassionate, patient-centered care in Port Jefferson. Qualifications

  • Current New York State Physician Assistant license.
  • NCCPA certification.
  • Excellent clinical, communication, and interpersonal skills.
  • Experience with EMR systems preferred.

Responsibilities

  • Perform comprehensive patient evaluations, including histories and physical exams.
  • Diagnose and manage acute and chronic conditions.
  • Develop and implement individualized treatment plans with supervising physicians.
  • Prescribe medications and order/interpret labs and tests as allowed by NYS.
  • Educate patients on prevention and healthy lifestyles.
  • Coordinate care with specialists and other providers.
  • Maintain thorough EMR documentation.
  • Collaborate with physicians, nurses, and staff to deliver team-based care.

Responsibilities
  • Perform comprehensive patient evaluations, including medical histories, physical examinations, and preventive health assessments.
  • Diagnose and manage acute and chronic medical conditions across a diverse patient population.
  • Develop and implement individualized treatment plans in collaboration with supervising physicians.
  • Prescribe medications and order, interpret, and review laboratory and diagnostic testing in accordance with New York State regulations.
  • Educate patients on disease prevention, wellness, and healthy lifestyle choices.
  • Perform office-based procedures within scope of practice.
  • Maintain accurate and timely documentation using the electronic medical record (EMR).
  • Coordinate care with specialists and other healthcare providers to ensure comprehensive patient management.
  • Foster strong, trusting relationships with patients while delivering compassionate, patient-centered care.
  • Collaborate closely with physicians, nurses, and office staff to provide exceptional healthcare in a team-based environment.
