Job summary

Metropolitan Jewish Health System, Inc. seeks a Nurse Practitioner or Physician Assistant to provide and coordinate primary care for Advantage members residing in long-term care facilities or at home.

You will develop care plans, ensure preventive services align with health outcomes, and act as a clinical resource to participating providers and facilities. This position emphasizes geriatric care and managed care coordination.

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • NYS RN/NP or PA License, Registration, DEA certificates.
  • 1–2 years clinical experience as NP/PA, including geriatric care.
  • Experience in managed care a plus.
  • Proficiency in EMR, case management software, Word and Excel.

Responsibilities

  • Provide and coordinate primary clinical care for assigned Nursing Home Residents.
  • Act as resource and contact for member/designated representative regarding care plans.
  • Act as team member and clinical resource to participating providers/facilities.
  • Collaborate with physicians to ensure preventive care and care planning.

MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. As a not-for-profit organization, many of our programs and services are made possible through the generosity of grateful families, corporate donors and grants, as well as our own employees.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantagemembers who reside in participating long term care facilities. You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives and you will act as clinical resource to the participating providers and facilities.

  • Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents
  • Acts as resource and contact for member/designated representative regarding all aspects of Advantage member care plan
  • Acts as team member and clinical resource to participating providers/facilities
  • Acts as resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program required.
  • NYS RN/NP or PA License, Registration, DEA certificates.
  • One to two years clinical experience in role of nurse practitioner or physician assistant required including 1 year in a geriatric specialty.
  • Previous clinical experience in acute, subacute and/or long term care setting preferred.
  • Prior experience in managed care a plus.
  • Proficiency in computer skills, electronic medical record, case management software, word, excel required.
