The position emphasizes value-based care, accountability for patient outcomes, and collaboration within a multidisciplinary team.**Primary duties and responsibilities*** **Comprehensive patient care**: Provide care for a panel of patients with multiple co-morbidities, focusing on prevention, diagnosis, and treatment of acute and chronic conditions. Prevention of unnecessary ER visits and Hospitalizations.
