Job summary

Pc CFL in Central Florida is seeking an experienced Family Medicine/Internal Medicine/General Practice physician to join a geriatric-focused team delivering comprehensive primary care for older adults across skilled nursing facilities, assisted living communities, and homes.

You will evaluate and treat geriatric patients, coordinate care with a collaborative clinical team, and educate families, with a mission-driven, physician-led environment that supports patient-centered outcomes.

Qualifications

• Board-certified or board-eligible in Family Medicine, Internal Medicine, or General Practice.
• Active, unrestricted Florida medical license.
• Current DEA license.
• Proven clinical experience and strong clinical judgment.
• Ability to build relationships with patients, families, and facility staff.

Responsibilities

• Evaluate, diagnose, and treat assigned geriatric patients.
• Develop individualized treatment plans and monitor patient conditions.
• Coordinate care with specialists and other healthcare professionals.
• Educate patients and families on health maintenance and disease prevention.
• Maintain accurate and timely medical records and documentation.
• Communicate effectively with patients, families, facility staff, and the care team.

The Opportunity

As a physician on our team, you will provide comprehensive primary care to geriatric patients while developing meaningful, long-term relationships with patients, families, and facility teams.

You will care for patients with complex medical needs while working collaboratively with our clinical team to coordinate care and support the overall health and well-being of each patient.

Your responsibilities will include:

• Evaluating, diagnosing, and treating assigned patients
• Developing individualized treatment plans
• Monitoring patient conditions and adjusting treatment as needed
• Coordinating care with specialists and other healthcare professionals
• Educating patients and families regarding health maintenance and disease prevention
• Facilitating referrals and transitions of careMaintaining accurate and timely medical records
• Communicating effectively with patients, families, facility staff, and members of the care team
• Building strong relationships with patients and the facilities we serve

What We're Looking For

• Board-certified or board-eligible physician in Family Medicine, Internal Medicine, or General Practice
• Active, unrestricted Florida medical license
• Current DEA license
• Proven clinical experience
• Strong clinical judgment and problem-solving skills
• Ability to develop strong relationships with patients, families, and facility staff
• Familiarity with electronic medical records and standard medical technology
• Bilingual skills are a plus
