Job summary

Christ Community Health Services seeks a compassionate Physician (MD/DO) to join our team in a faith-based FQHC setting in Jackson, TN. The role emphasizes whole-person primary care within our Christian mission, serving underserved communities and coordinating with an integrated care team.

The physician will provide evidence-based care across the lifespan, participate in quality initiatives, and support access through same-day visits and telehealth.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible.
• Active, unrestricted Tennessee medical license.
• Current DEA registration.
• Current BLS certification.
• Primary care experience preferred.
• Experience in community health/FQHC or with underserved populations preferred.
• Strong commitment to patient-centered, team-based care.

Responsibilities

• Provide evidence-based primary care to patients across the lifespan.
• Diagnose, treat, and manage acute and chronic medical conditions.
• Promote preventive care, wellness, and population health.
• Coordinate care with specialists, behavioral health, pharmacy, and other services.
• Partner with patients through shared decision-making and individualized care plans.
• Address patients' physical, emotional, social, and spiritual needs in a faith-based setting.
• Maintain accurate medical documentation and coding.
• Participate in care-gap closure and quality improvement initiatives.
• Support patient access via same-day visits, telehealth, extended hours.
• Participate in provider meetings and interdisciplinary discussions.
• Support training of students, residents, and new providers.
• Engage in community outreach and engagement activities.
• Maintain compliance with HRSA, UDS, PCMH, regulatory, safety, and organizational standards.

What You'll Do

As a Primary Care Physician, you will:

• Provide evidence-based primary care to patients across the lifespan.
• Diagnose, treat, and manage acute and chronic medical conditions.

• Participate in care-gap closure, quality improvement, peer review, and other FQHC initiatives.
• Support patient access through same-day visits, telehealth, extended hours, and other access initiatives as needed.
• Participate in provider meetings, team huddles, case conferences, and interdisciplinary discussions.
• Support the training and development of students, residents, and new providers when applicable.
• Participate in community outreach and engagement activities.
• Maintain compliance with HRSA, UDS, PCMH, regulatory, safety, and organizational standards.

This position is based in a clinical environment serving patients of all ages and may involve exposure to infectious or contagious illnesses, bloodborne pathogens, bodily fluids, and sharps.

The role involves a combination of sitting, standing, and walking throughout the clinical day, including patient examinations, procedures, documentation, chart review, and movement between clinical areas. Occasional travel between clinic locations, community outreach events, or organizational meetings may be required.
