Responsibilities

  • Provide patient and facility confidentiality at all times.
  • Treat patients, visitors and coworkers with respect, dignity and kindness.
  • Provide quality care for residents within scope of capabilities.
  • Complete facility orientation within 14 days of employment including health screen.
  • Complete departmental orientation and skills checklist within 90 days of employment.
  • Participate in mandatory staff in-services and quality improvement studies.
  • Maintain regular and consistent attendance as an essential function.
  • Comply with all facility safety requirements to ensure a safe working environment.
  • Follow the patient’s therapy plan of care including ROM, exercises, gait training.
  • Record therapies given and document patient responses.

Essential Job Duties and Responsibilities
  • Maintain patients and facility confidentiality at all times.
  • Treat patients, visitors and coworkers with respect, dignity and kindness.
  • Provide quality care for our residents to the utmost of our abilities and within the scope of our capacities.
  • Complete the general facility orientation program within 14 days of employment including health screen, physical and immunizations.
  • Complete the departmental orientation program and skills checklist within 90 days of employment.
  • Participate in mandatory staff in-services and quality improvement studies.
  • Regular and consistent attendance is an essential function of the job. Employees are expected to report to work on time and as scheduled.
  • Comply with all facility safety requirements to ensure a safe working environment for all employees.
  • Perform specific job duties in the most economical manner possible.
  • Create and maintain an atmosphere of warmth, interest and positive emphasis, as well as calm environment throughout the facility. Employees must conduct themselves in a manner that is conducive to our company’s operational policies and procedures.

  • Perform other job-related tasks as required or requested by supervisor or administrator.
Duties and Responsibilities
  • Provide and carry out one-to-one programming interventions, sensory stimulation, friendly visits, set-up of leisure pursuits and any additional instructions which will meet the individual needs for each resident as assessed.
  • Announces daily devotions and remaining activities of the day.
  • Collect news and take photographs for local newspaper.
  • Assists with transporting residents to and from activities in the facility, as well as scheduled outings in the community.
  • Reports any abnormal signs or symptoms which signify a physical change in condition which would require nursing intervention.
  • Participates in Quality Assurance activities as assigned by the Quality Assurance Coordinator.
  • Other duties as assigned by the supervisor.

  • Follow the patient’s therapy plan of care, does or assists with the following:
    • Range of motion exercises
    • Exercises
    • Gait training using walkers, canes, and other assistive devices as instructed
    • Provides hot packs
    • Positioning
    • Other treatment modalities as instructed by the Physical/OT/Restorative or Therapy Aide Supervisor
    • Records all therapies given
    • Documents patients response to therapy
    • Assists nursing staff with transfers when required
    • Uses safety straps and other protective devise and measures to protect the resident and self from injury
Qualifications

High school diploma or general education degree (GED), preference will be given to applicant with associates or higher degree. One to three years of related experience is preferred
