Job summary

Okeena Health is seeking a Physical Therapist (PT) per diem/PRN to join our Therapy Team in Tennessee. You will plan, organize, develop, and direct Physical Therapy services in line with current standards while maintaining high-quality resident care.

In this role you will provide PT services, document and bill per procedures, evaluate residents, and develop treatment plans with clear goals. The facility offers competitive pay, flexible scheduling, PTO, 401K, and benefits.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in TN.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Evaluate residents to inform treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate findings and plans to residents, families, and team members.
• Complete CMS-compliant documentation in NetHealth EMR.

Okeena Health and Rehabis hiring for a Physical Therapist PT to join our Therapy Team to assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Status: Per Diem/PRN

Job Functions:

• Provide physical therapy services to residents in accordance with established standards of practice, Company procedures, Therapy Department procedures and productivity standards.
• Adhere to Company procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents at regular intervals to determine need for intervention/treatment.
• Conduct timely screening of residents referred to physical therapy to determine need for intervention/ treatment.
• Evaluate residents to obtain data necessary for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans by establishing short and long-term goals and methods to achieve identified goals.

• Interpret and communicate evaluation findings, treatment plan and recommendations to residents, families/ responsible parties, and interdisciplinary team members.
• Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization.
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

Education & Requirements:

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program
• Must have an active Physical Therapy license in TN

• Ongoing continuing education and professional development to maintain current licensure and certification

• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.
