Mercyhealth Visiting Nurses Association, Inc. is seeking a licensed Physical Therapist to provide home health services in Illinois/Wisconsin. You will evaluate patients, design and implement individualized treatment plans, and monitor progress across care settings.
The role emphasizes manual therapy, therapeutic exercises, and use of assistive devices, with family/caregiver education and coordination with physicians to ensure seamless care.
Graduate of an accredited Bachelor, Master or Doctorate program for Physical Therapy Home Health experience is preferred
License to practice Physical Therapy in the state of Wisconsin and/or Illinois as required
Approved CPR or BLS certification required within 90 days of hire
Passing the Driver’s License Check and/or Credit Check (for those positions requiring).
Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.
Must be able to follow written/oral instructions.
