Qualifications

• Requires an Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist in evaluation planning and target setting with the Physical Therapist.
• Carry out prescribed treatment plans including mobility, balance, strengthening, ROM and modalities.
• Assist in documenting compliant and medical necessary CMS CMS in NetHealth EMR.
