Job summary

Suburban Woods Health & Rehabilitation in Pennsylvania is seeking a licensed Physical Therapy Assistant to help residents regain independence in a skilled nursing facility. You will implement therapy plans and guide patients through exercises to meet goals set with the therapy team.

Qualifications

• Must have an Associate’s Degree in a Physical Therapy Assistant program from an accredited college or university.
• Must possess a current, unencumbered license to practice as a Physical Therapy Assistant within the state in which you work.
• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Responsibilities

• Return resident to optimal physical function and facilitate achievement of patient and therapy goals in a SNF setting.
• Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification.
