Job description

Colavria Therapy Services provides innovative and quality rehabilitation to our 5-star rated communities: The Rehabilitation Center at Sandalwood and Westlake Care Community.
We are proudly managed by Colavria Hospitality, a local, family-owned company specializing in assisted living, rehabilitation, and long-term care in the greater Denver area.
What you’ll be doing every day:
  1. Collaborate with your team to enhance the effectiveness of physical therapy by identifying resident care issues, providing information and commentary, recommending options and courses of action, and implementing physical therapy directives.
  2. Assess resident health through interviews, physical examinations, and reviewing therapy histories.
  3. Review physical therapist evaluations and test results to assess abnormal conditions.
  4. Administer therapeutic procedures, including manual exercises, instructing and assisting residents with physical activities, and using supportive devices like crutches, canes, and prostheses.
  5. Educate and counsel residents on therapeutic regimens, growth and development, and promote wellness and health maintenance.
  6. Develop and implement resident management plans to ensure continuity of care.
Who we are looking for:
  • Individuals passionate about working with unwell, disabled, or dementia-affected populations, demonstrating patience, customer focus, and the ability to build trust and rapport with residents and team members.
  • Graduate of an accredited physical therapy program (APTA)
  • Registered or licensed in Colorado
  • One year of experience in long-term care or related settings preferred
