Job summary

Adaptive Home Health, LLC is expanding in Detroit and seeking a dedicated Physical Therapy Assistant (PTA) to deliver home health care. This field-based, part-time role supports patients in their homes with supervision from a PT, focusing on mobility, safety, and outcomes.

You will manage a high-quality caseload, document thoroughly, and collaborate with care teams to improve patient experiences and outcomes in the Detroit area.

Qualifications

• Active Michigan PTA license (in good standing).
• Reliable transportation and valid driver’s license.
• Home health experience strongly preferred.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PTA scope.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans with a focus on gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
• Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.
• Drive multidisciplinary coordination: elevate changes in condition, align on orders, and keep the care team moving.
• Coach patients and caregivers with practical home programs that actually get done.
• Document promptly and thoroughly — defensible, efficient, and consistent.

• Schedule: Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed) Minimum of 10 points per week.

• Compensation (Productivity Points): $58.00 / point | $75,000-$100,000/year

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a high-quality caseload: Minimum of 10 productivity points/week at target visit quality.

2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

Requirements

• Active Michigan PTA license (in good standing)

  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

  • Strengthening, functional mobility, and home safety assessment/modification education
