Job summary

Adaptive Home Health, LLC is seeking a Physical Therapy Assistant to perform in-home PTA visits under a supervising PT. The role emphasizes functional mobility, home safety, and measurable progress within PTA scope.

You will coordinate with the care team, document thoroughly, and maintain flexibility to meet patient needs across a field-based territory. Colorado PTA license required, with strong communication and independent practice expected.

• Execute treatment plans focused on gait/balance, transfers, strengthening, home safety, fall prevention.
• Set and track objective progress; communicate updates to supervising PT.
• Drive multidisciplinary coordination and keep the care team aligned.

• Role: Physical Therapy Assistant (PTA)

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field Based

• Compensation (Productivity Points): $58.00 / point | $75,000-$100,000/year

The PTA role is a core part of that mission: you help restore function, keep patients safe at home, and drive real outcomes — in partnership with a supervising PT.

We also build technology to support field clinicians. Our charting product reduces documentation burden, and our scheduling product (launching soon) will make routing and visit coordination easier — protecting your time.

What you will do:

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.

• Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

• Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT.

• Drive multidisciplinary coordination: elevate changes in condition, align on orders, and keep the care team moving.

• Coach patients and caregivers with practical home programs that actually get done.

• Document promptly and thoroughly — defensible, efficient, and consistent.

Must-haves

• Active Colorado PTA license (in good standing)

• Strong clinical judgment and comfort working independently in the field (within PTA scope and under PT supervision)

• Comfortable escalating changes in condition to PT/RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on treatment plans, visit cadence, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health PTA scope

  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device education

  • Strengthening, functional mobility, and home safety assessment/modification education

  • Patient/caregiver coaching with practical home programs

Nice-to-haves

• Prior home health experience

• Deeper clinical experience in high-signal ortho/neuro populations

• Strong organization and time management
