Position: Physical Therapy Assistant for an In-house Skilled Nursing Facility
License Requirement: Must have active Wisconsin Physical Therapy Assistant license

Job Type: PRN

Job Purpose

Assist in planning, organizing, developing, and directing Physical Therapy services to ensure high-quality resident care, following applicable standards and regulations.

Key Responsibilities

• Provide physical therapy services under the supervision of the Physical Therapist.
• Conduct resident screenings to determine intervention needs.
• Assist in evaluations and treatment planning.
• Perform individual and group treatments, including exercises, mobility, balance, gait training, and modalities.
• Monitor residents' responses and modify treatments as needed.
• Train residents, families, and staff on equipment use.
• Participate in resident, family, and staff education.
• Complete documentation in accordance with CMS and EMR standards.

Qualifications

• Associates Degree in Physical Therapy from an accredited program.
• Active Wisconsin Physical Therapy Assistant license.
