Location: Madison, Wisconsin

Type: PRN, Full-time

Pay Range: $35.00/hr

Evolve Therapy Services is a therapy management organization seeking compassionate Physical Therapy Assistants for our affiliated SNF facility, Champion Care.

Job Purpose

Assist in planning, organizing, developing, and directing Physical Therapy services to ensure high-quality resident care, in accordance with applicable standards and regulations.

Job Functions
  • Provide physical therapy services under the supervision of a Physical Therapist.
  • Conduct resident screenings and assist in evaluations.
  • Help develop and implement treatment plans.
  • Perform individual and group treatments, including muscle strength, mobility, balance, gait, and range of motion activities.
  • Monitor residents' responses and modify treatments as needed.
  • Assist in resident scheduling and staff training.
  • Complete documentation according to CMS standards using NetHealth EMR.
Requirements
  • Associates Degree in Physical Therapy from an accredited program.
  • Active Wisconsin Physical Therapy Assistant license.
  • Ongoing continuing education to maintain licensure, with unlimited CEU access via MedBridge.
