Job Description

Evolve Therapy Services is a therapy management organization hiring compassionate therapists for our partner located in S. Pasadena, FL.

Position: Physical Therapy Assistant (In-house Skilled Nursing Facility)

Job Type: Part-Time or PRN

Purpose: To assist in planning, organizing, developing, and directing Physical Therapy services to ensure high-quality resident care, in accordance with applicable standards and regulations.

Responsibilities:

• Provide physical therapy services under the supervision of a Physical Therapist.
• Conduct resident screenings and assist with evaluations.
• Assist in developing and implementing treatment plans.
• Conduct individual and group treatments, including strengthening, mobility, balance, gait, and modalities.
• Select and adapt therapy techniques/media based on resident performance.
• Monitor resident responses and modify treatments as needed.
• Assist in scheduling and resident education.
• Document all procedures in compliance with CMS and using NetHealth EMR systems.

Qualifications:

• Associates Degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in the state of practice.
• Commitment to ongoing continuing education and professional development.
