Job summary

VitalCaring Group is seeking a Physical Therapy Assistant (PTA) to deliver high-quality, patient-centered care in patients’ homes. You will work one-on-one with individuals to improve strength, mobility, and independence, following plans of care under a licensed Physical Therapist.

Ideal candidates have PTA certification, CPR, and a valid driver’s license. Home health experience is a plus, with opportunities for growth and comprehensive benefits in a supportive field-based team.

Qualifications

• Graduate of an accredited PTA program.
• Current PTA license or state certification as required.
• 1+ year of experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license with reliable transportation and auto insurance.

Responsibilities

• Provide direct PTA services under the supervision of a licensed Physical Therapist.
• Follow individualized plans of care to reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.

What You’ll Do

Deliver High-Quality Patient Care

• Provide direct physical therapy services under the direction of a licensed Physical Therapist
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility
• Support patients with transfers, strengthening, and activities of daily living
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate
• Educate patients and caregivers on exercises, safety, and use of assistive devices

Monitor Progress & Communicate

• Observe and report patient progress, response to treatment, and any changes in condition
• Maintain accurate, timely documentation in the EMR
• Communicate effectively with the Physical Therapist and interdisciplinary care team
• Participate in case conferences and care coordination efforts
• Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care
• Participate in team meetings, in-services, and ongoing training
• Uphold clinical standards and contribute to quality improvement efforts

What You Bring
