Responsibilities

• Provide home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury
• Collaborate with a home health team including RN, LPN, CNA, HHA, OT, and SLP, focusing on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress, and modify care plans as needed
• Assist patients with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their care plan, goals, exercises, and self-care
• Coach family members on the plan of care, therapy goals, and safe exercise assistance

Minimum Qualifications

• Graduate of an accredited Physical Therapist Program and active PTA license in Wisconsin
• Minimum of 1 year of physical therapy assistant experience, preferably in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Strong clinical judgment, interpersonal skills, dexterity, and compassion
