Job summary

Preferred Care Home Health is seeking a Physical Therapist Assistant (PTA) for home health in Naples, FL. You will deliver skilled therapy under a licensed PT, guide patients through exercises, and document progress in coordination with a multidisciplinary team.

The role requires PTA program graduation, an active PTA license in Florida, CPR certification, and a valid driver’s license. Home health or rehab experience is a plus; strong communication and organizational skills are essential.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active PTA license in the state of practice.
• Current CPR certification.
• Valid driver's license with reliable transportation.
• Home health, rehab, or skilled nursing experience a plus.
• Strong communication, organizational, and documentation skills.

Responsibilities

• Deliver skilled physical therapy under the direction of a licensed PT and in line with the physician's plan of care.
• Guide patients through therapeutic exercises to build mobility, strength, balance, and function.
• Track and document patient progress toward treatment goals.
• Teach patients and caregivers safe techniques and home exercise programs.
• Complete clinical notes and progress reports accurately and on time.
• Collaborate with physicians, therapists, nurses, and caregivers as part of a coordinated care team.
• Take part in case conferences and care planning as needed.
