Home Health Links is seeking a Physical Therapist Assistant to treat patients in the Home Health setting. Our company is Therapist-owned and operated so we understand what it takes to be in the field.

What you do:

• Follow the plan of care by Physical Therapist and provide treatment to patients
• Interpret evaluation findings to provide care and safety relating to dysfunction
• Provide treatment based on the goals and follow through with treatment to achieve established goals
• Provide advice and education to patients and family
• Regularly assess clinical signs and symptoms to determine the effectiveness of treatment, the progress of the patient towards goals, and the need for modifying plans and treatment
• Provide discharge planning for patients from physical therapy and communicate when the patient has received maximum benefit from occupational therapy
• Provide accurate, timely documentation of physical therapy patient management and demonstrate progress toward meeting the criteria for professional practice

We set standard base pay ranges for all roles based on function and level. The final offer is determined by multiple factors, including skillset, work experience, and languages, and may vary from the amounts listed below.

The pay range for this position is: $60-$75/visit

• Graduate of accredited Physical Therapist Assistant Program
• Home Health experience preferred; not required
