Job Purpose

Provide physical therapy treatments to PruittHealth Home Health patients in their residence under the supervision/direction of a physical therapist.

Key Responsibilities

1. Assist with the development and implementation of the plan of care within goals defined by the physician, patient/patient family, and interdisciplinary team.
2. Help maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan.
3. Demonstrate intermediate knowledge of home care and competency in discipline-specific patient care skills.
4. Meet agency productivity standards and utilize time and resources effectively.
5. Complete documentation timely, accurately, and at the point of care, according to industry standards.
6. Assist the physical therapist in assessing the physical disability and level of function of patients.
7. Maintain current license, certification, and clinical records to meet regulations and guidelines.
8. Participate in surveys (Licensure / JCAHO) and required reports.
9. Report any alleged or suspected violations to supervisor and/or others until resolved.
10. Be self-directed, prioritize, manage time appropriately, and meet outcomes and deadlines.

Qualifications

Licensure, Certification, and Education Requirements

• Valid and unrestricted professional license in the state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• Foreign-trained candidates must have their program deemed equivalent by a credentials evaluation organization approved by the APTA.

• Minimum of 2 years' therapy experience.
• Valid driver’s license and automobile liability insurance.

Preferred Skills and Experience

• Prior Home Health experience is preferred.
