Job description

Overview

Implements plan of care as provided by the physical therapist, treating patients of all ages. Serves as a role model in delivery of professional services and as a clinical resource for staff and students.

Qualifications
  • Certifications: Licensed Physical Therapist Assistant (PTA) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
  • Education: Associate's Degree in Physical Therapy Assistant (Required)
Responsibilities
  • Educates patients, family, caregivers, students, and other healthcare providers using relevant and effective teaching methods.
  • Follows the therapist's plan of care. Adjusts treatment based on patient response and evidence after communicating with the evaluating therapist and documenting in the daily note.
  • Produces documentation in accordance with the Clinical Documentation Policy (CTSCP67).
  • Completes annual education as assigned to comply with DNV Hip and Knee Replacement Program requirements, if applicable.
  • Implements intervention plans according to the established treatment plan, utilizing functional activities considering the patient's needs. Modifies approach based on patient behavior and performance. Uses standardized outcome measures to reflect functional performance accurately. Documents in accordance with regulatory and organizational standards.
  • Collaborates with patients, families, and significant others throughout rehabilitation, providing education and family training as necessary.
  • Communicates with supervising Occupational Therapist regarding changes in patient status, performance, and treatment plan adjustments. Clearly communicates patient performance, status changes, activity/transfer recommendations, discharge plans, and equipment needs to the medical team.

Compensation and Benefits
