Job Position: Physical Therapy Assistant for an In-house Skilled Nursing Facility.

Job Type: PRN.

Job Purpose: To assist in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by the Administrator, to ensure the highest quality resident care.

Job Functions:

  1. Provide physical therapy services to residents under the supervision of the Physical Therapist.
  2. Conduct screenings of residents regularly to determine the need for intervention/treatment.
  3. Identify indications for evaluations at appropriate intervals and assist in evaluations under the direction of the Physical Therapist.
  4. Assist the Physical Therapist in setting short-term and long-term goals and treatment plans.
  5. Conduct individual and group treatments as assigned, including muscle strength, coordination, mobility, sensory awareness, transfer training, balance activities, gait training, range of motion, and modalities.
  6. Assist in selecting and adapting physical therapy techniques/media based on resident performance and consultation with the Physical Therapist.
  7. Monitor residents’ responses to interventions and consult with the Physical Therapist for modifications.
  8. Help establish and adjust individual resident schedules in collaboration with the Physical Therapist and team members.
  9. Adhere to department and resident schedules, modifying as needed for treatment regimens.
  10. Train residents, families, and staff in equipment use or application.
  11. Participate in education for residents, families, and staff.
  12. Demonstrate sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
  13. Report to work punctually and coordinate schedules for maximum productivity.
  14. Assist with Quality Assurance programs as required.
  15. Complete compliant and medically necessary documentation per CMS expectations using NetHealth EMR systems.

Education & Requirements:

  • Associates Degree in Physical Therapy from an accredited program.
  • Maintain ongoing continuing education and professional development.
  • Hold an active Physical Therapy Assistant license in practice state(s).
  • Ensure ongoing licensure and certification through continuing education.
