Optimal Care is seeking a dedicated Physical Therapy Assistant to provide in-home and community-based PT services in Grand Rapids, MI. You will deliver interventions under the supervision of a physical therapist, support patient mobility, and educate families on exercises and DME use.

The role requires patient-centered care, accurate documentation, and adherence to physician plans of care. Full-time schedule, 8:00 am–5:00 pm, with weekend rotation; pay range $30–$34 USD per hour plus benefits

Qualifications

• Successful completion of a Physical Therapy Assistant program approved by the American Physical Therapy Association (APTA).
• Licensed PTA in the state where practicing.
• Ability to care for adult and geriatric populations; effective communication.
• Strong documentation and EMR skills.
• Reliable transportation and valid auto insurance.

Responsibilities

• Provides direct PT services in accordance with physician/therapist orders.
• Assists patients with therapeutic exercises, gait training, and functional activities.
• Instructs patients and families in home programs and use of DME.
• Maintains accurate progress notes and treatment records.
• Observes patient response and reports changes to supervising therapist.

Key Responsibilities

The Physical Therapy Assistant administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician or advanced practice practitioner’s orders and plan of care under the direction and supervision of the Home Health leadership team.

In this role you will be responsible for:

• Provides direct physical therapy services planned, delegated and supervised according to directions of physical therapist and in accordance with the physician’s plan of care.
• May treat patient to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait, and functional training, transfer activities and prosthetic training.
• Utilizes physical agents, including but not limited to heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.
• Instructs patients and families in care and use of DME.
• Maintains necessary records; clinical, conference, and progress notes, which will be incorporated into the patient’s clinical record in prescribed time frames

Required Qualifications

• Successful completion of a physical therapy assistant program at a school of physical therapy approved by the American Physical Therapy Association
• Physical Therapy Assistant License recognized in the state in which practicing
• Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
• Demonstrates knowledge of the principles of growth and development over the life span
• Able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirements relative to their age-specific needs
• Demonstrates computer skills sufficient to use the electronic medical record effectively
• Reliable transportation and valid automobile insurance

Desired Qualifications

• 2 years experience working as a Physical Therapy Assistant - hospital and/or home care experience preferred

Pay Range

$30—$34 USD
