Job summary

Optimal Care - Evergreen is seeking a Physical Therapy Assistant to deliver patient-focused care in home health settings in Jackson, MI. You will work under a PT, execute treatment plans, and support geriatric patients in their homes.

The position emphasizes mobility, pain relief, and functional restoration, with emphasis on team collaboration, documentation, and adherence to plans of care. Travel to patient residences is required.

Qualifications

  • Must complete PTA program approved by APTA
  • PTA license in the practicing state
  • Ability to assess client status and plan individualized care
  • Computer skills to use EMR effectively
  • Reliable transportation and automobile insurance

Responsibilities

  • Provide PT services as directed by the PT and plan of care
  • Treat patients to relieve pain, restore function, and maintain performance
  • Guide patients in therapeutic exercises and mobility training
  • Utilize modalities (heat, cold, ultrasound, massage)
  • Observe and report patient responses to treatment
  • Instruct patients/families in use of DME
  • Maintain patient records and progress notes in EMR timeline

Key Responsibilities

The Physical Therapy Assistant administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician or advanced practice practitioner’s orders and plan of care under the direction and supervision of the Home Health leadership team.

In this role you will be responsible for:

  • Provides direct physical therapy services planned, delegated and supervised according to directions of physical therapist and in accordance with the physician’s plan of care.
  • May treat patient to relieve pain, develop or restore function, and maintain maximum performance.
  • Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait, and functional training, transfer activities and prosthetic training.
  • Utilizes physical agents, including but not limited to heat, cold, water, light, electricity, ultrasound and massage.
  • Observes and reports to the physical therapist and the Organization personnel the patient's reaction to treatment and any changes in patient's condition.
  • Instructs patients and families in care and use of DME.
  • Maintains necessary records; clinical, conference, and progress notes, which will be incorporated into the patient's clinical record in prescribed time frames
Required Qualifications
  • Successful completion of a physical therapy assistant program at a school of physical therapy approved by the American Physical Therapy Association
  • Physical Therapy Assistant License recognized in the state in which practicing
  • Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
  • Demonstrates knowledge of the principles of growth and development over the life span
  • Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
  • Demonstrates computer skills sufficient to use the electronic medical record effectively
  • Reliable transportation and valid automobile insurance
Desired Qualifications
  • 2 years experience working as a Physical Therapy Assistant-hospital and/or home care experience preferred

  • Main Service Area: Jackson and surrounding
Hours
  • 8:00 am – 5:00 pm, Monday through Friday

Pay Range

$30 - $33 USD
