Job description

Responsible for carrying out the care plan as designed by the Physical Therapist (PT) and assisting the PT during evaluations. Monitor residents' progress and communicate with PT. Maintain documentation in the system of residents' activities, record progress, and document billing activities. Perform residents' home assessments as part of the discharge process as directed by the PT. Conduct residents' screenings during admission, quarterly, and annual assessments to determine the need for therapy, as directed by the PT.

Requirements:

1. Associate Degree from an APTA-accredited program (Required)
2. Current State of Florida PTA license (Required)
3. Two years of experience in a healthcare setting (Preferred)
