Job summary

Live Well Homecare is seeking a Physical Therapy Assistant to collaborate with physical therapists to implement programs that restore mobility, strength, coordination, and range of motion across the continuum of care, with a focus on quality and patient satisfaction.

You will document sessions, educate patients and families, coordinate with care teams, and uphold infection prevention, confidentiality, and professional standards while contributing to ongoing professional development.

Qualifications

• Associates Degree in Physical Therapy required or equivalent.
• Licenses and certifications: PTA license and BLS or ARC-BLS certification required upon hire.
• Strong communication, customer service focus, teamwork, and basic computer skills.

Responsibilities

• Implement treatment plans under therapist supervision, selecting appropriate activities to meet patient goals.
• Adjust treatment plans collaboratively with therapists and patients/families.
• Provide patient and family education, discharge instructions, and referrals to community resources.

• Maintain ethical standards, confidentiality, punctuality, and professionalism, including effective communication and teamwork.
• Promote a positive environment, demonstrate excellent customer service, and apply service recovery principles.
• Participate in ongoing professional development, mandatory training, and orientation activities.
• Adhere to organizational policies, standards of conduct, and infection prevention protocols.
• Perform additional duties as assigned.
