Hope At Home Health Care, LLC is a rapidly growing company that is looking to add experienced Physical Therapy Assistants to our team who are ready to make a difference in the lives of the patients that trust us with their care.

Duties

  • Assists the physician in evaluating patient’s level of function.
  • Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests.
  • Treats patients to relieve pain and/or restore function to maintain maximum performance.
  • Makes arrangements for out patient services, which cannot be given in the home.
  • Maintains communication with the Nurse /Case Manager.
  • Records and reports to the physician the patient’s reaction to treatment and/or any changes of the patient’s condition.
  • Instructs patients and their families in the use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, etc.).
  • Instructs the family in the patient’s total physical therapy program.
  • Attends PTA staff meetings and in-service presentations.
  • Participates in care conferences.
  • Participates in staff and Agency development activities.
  • Prepares an assessment of the patient’s condition and periodic evaluations and summaries and writes clinical/progress notes on the date of the visit and incorporates same in the patient record no less than weekly.
  • Participates in the development and periodic revision of the physician’s Plan of Care.

QUALIFICATIONS

  • Registered by the State of Michigan
  • Current Michigan driver’s license and/or reliable transportation with proof of automobile insurance
  • CPR certification preferred
