Innovate Rehab & Wellness in Brookfield, MO is hiring a Licensed PTA for a Skilled Nursing Facility PRN role. You will work under a licensed Physical Therapist to help patients improve mobility, regain strength, and increase function through individualized treatment plans.

Qualifications

• Associate degree from an accredited PTA program.
• Valid state PTA license.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
• Maintain accurate, compliant therapy records.
• Collaborate with the care team for coordinated treatment.

Education

Associate degree from an accredited PTA program

Valid state PTA license

Rehab/clinical experience preferred

Strong communication and interpersonal skills

CPR certification preferred
