Job summary

Northwell Health is seeking a capable Physical Therapy Assistant to assist in administering therapy care programs and monitor patient progress. You will maintain medical records and communicate progress to the Physical Therapist and other health personnel.

The role includes documenting care, participating in quality improvement, and possibly attending family meetings to discuss patient welfare and care plans. NYS licensure or limited permit is required, and outpatient settings may apply.

Qualifications

• Associate's Degree or equivalent required.
• Active NYS license as Physical Therapy Assistant required.
• Limited permit to practice in outpatient settings may be required.

Responsibilities

• Provide direct patient care under supervision per prescriptions.
• Submit documentation per department policies and procedures.
• Maintain progress notes and discharge summaries.
• Respond to department emergency procedures for patient safety.
• Participate in quality improvement chart reviews and activities.
• Attend family meetings pertaining to patient welfare as needed.
• Perform related duties; essential functions per ADA guidelines.
