Job summary

Northwell Health is seeking a Physical Therapy Assistant to assist in administering therapy care programs and monitor patient progress under a Physical Therapist. You will maintain patient records and communicate status with the care team.

Responsibilities include direct patient care, documentation, and participation in quality improvement activities. NY PTA licensure is required, with a limited permit allowed for outpatient settings, and 1–3 years of related experience preferred.

Qualifications

• Associate's Degree required.
• Current License to practice as a Physical Therapy Assistant in New York State, required.
• Current limited permit to practice as a Physical Therapy Assistant in New York State, required (outpatient settings).
• Specialized certifications, as needed.
• 1-3 years of relevant experience, required.

Responsibilities

• Administers direct patient care as prescribed by the referring physician or state-authorized practitioner.
• Submits required documentation per department policies and procedures.
• Maintains progress notes and discharge summaries for assigned patients.
• Responds to department emergency procedures for patient safety and care as needed.
• Participates in quality improvement chart reviews and activities.
• May attend family meetings pertaining to patient welfare and management.
• Performs related duties; essential functions as per ADA.
