FHCSD’s Program for All-inclusive Care for the Elderly (PACE) is designed to help our patients maintain a high standard of independent living. Our program provides a wide range of supportive services including medical care, meals catered to dietary needs, medical equipment, transportation, social services, and social activities. We are currently seeking a Licensed Physical Therapist Assistant to provide care for our participants as part of an interdisciplinary team.

Job Duties

Work with interdisciplinary team to create and implement custom care plans
Evaluate patient's progress and results of physical therapy
Recommending adjustments and modifications to supervising Physical Therapists as needed
Help patient develop or regain physical and/or mental functioning
Assist participants with adjusting to disabilities
Promote maximum independence by selecting and constructing therapies according to the individual's physical capacity, intelligence level, and interests
Adhere to the treatment plan and work within the scope of practice at all times
Work effectively as a member of the treatment team
Effectively communicate with supervising Physical therapists, Case Managers, and Scheduling staff
Refer participants to internal and external providers in a timely and appropriate manner
Complete all required documentation in accordance with department standards
Performs other duties as assigned

Requirements:

California State licensure as a Physical Therapy Assistant.
Graduation from an accredited Physical Therapy Assistant program.
Experience in outpatient clinic preferred.
Current American Heart Association healthcare provider CPR (BLS), Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.

$32.00 - $37.54

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
