Job summary

Preceptor Home Health & Hospice is seeking a licensed Physical Therapist Assistant (PTA) in Milwaukee County for a full-time position. The PTA will deliver in-home physical therapy services, implement treatment plans, and promote patient independence and safety.

Ideal candidates are licensed in Wisconsin, CAPTE-accredited program graduates, and have passed the national PTA exam. A valid driver’s license and vehicle are required to serve patients in their homes.

Qualifications

• Wisconsin PTA license required.
• Graduate of a CAPTE-accredited PTA program.
• Successful completion of the national PTA exam.

Responsibilities

• Providing hands-on physical therapy treatments following the established plan of care.
• Helping patients restore movement, reduce pain, and improve function.
• Guiding patients through therapeutic exercises, gait training, and mobility activities.
• Assisting with transfer techniques and prosthetic/orthotic training.
• Utilizing therapy modalities such as heat, cold, ultrasound, electricity, and massage.
• Monitoring patient progress and communicating changes to the care team.
• Educating patients on the safe use of assistive devices (walkers, canes, wheelchairs).
• Maintaining accurate and timely clinical documentation.
• Participating in team meetings and ongoing professional development.
