Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Webster, MA. You will provide PT services under the supervision of a Physical Therapist, participate in screenings, and contribute to treatment planning and resident goals.

Eligible candidates hold an Associate's degree in Physical Therapy and an active PTA license in MA, with a commitment to ongoing professional development.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in MA.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and in forming treatment plans with the PT.
• Assist in establishing resident schedules and treatment regimens.
• Educate residents, families, and staff on therapy processes.
• Document compliant and medically necessary information in NetHealth EMR.
• Participate in Quality Assurance activities.
