Job summary

Oak Glen Healthcare and Rehabilitation Center is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. The PTA will promote health and participate in life activities under a physician-approved plan of care, ensuring safety and effectiveness of the service delivery.

The position involves developing therapy programs, educating patients and caregivers, and coordinating with the interdisciplinary team to optimize patient outcomes and quality of life.

Qualifications

• Must hold an Associate degree from a CAPTE-accredited PTA program.
• Active state Physical Therapy license in good standing is required.
• Current CPR certification is required.
• At least 1 year experience in a Skilled Nursing Facility preferred.
• Ability to work independently and as part of a team.

Responsibilities

• Develop and implement individualized therapy programs to restore and enhance patient performance.
• Identify needs and refer to other disciplines to address comprehensive care.
• Instruct and educate patients and caregivers to promote independence and quality of life.
• Supervise PTA students and support personnel as per licensure rules.
• Develop personalized care plans using various treatment techniques.
• Provide evidence-based care using proven interventions.
• Promote healthy lifestyles by improving strength, flexibility, balance, and coordination.
• Monitor progress and communicate updates with the interdisciplinary team.
