Responsibilities
  • Provide professional rehabilitation therapy under the supervision of a licensed Physical Therapist to patients in their homes, implementing individualized, goal-oriented therapy plans in line with organizational policies.
  • Contribute to developing treatment programs based on patient and caregiver observations, providing ongoing treatment focused on measurable short and long-term goals, and monitoring effectiveness for necessary adjustments.
  • Build rapport with patients to create an environment conducive to goal achievement. Execute treatment plans using appropriate clinical skills while ensuring safety in the home setting. Focus on patient and caregiver engagement with goals and discharge plans.
  • Document all treatment activities per organizational policy, including communication and supervisory visits with the Physical Therapist, ensuring records support the need for continued skilled care.
  • Educate patients and caregivers effectively, providing clear instructions and written home exercise programs, and document treatments accurately in the EMR.
  • Follow the physician’s plan of care regarding service frequency and duration, and act as a liaison among healthcare team members to ensure coordinated care and resource referrals.
  • Report patient concerns promptly, participate in discharge planning, and adapt treatment as needed to meet goals.
  • Review PT evaluations with supervising therapists before initial visits, and participate in team meetings, training, and caseload reviews.
  • Assist with various DME devices, including wheelchairs, lifts, splints, and other mobility or orthopedic equipment.
  • Adhere to scope of practice, respond to messages promptly, and ensure accurate scheduling and productivity.
  • Demonstrate flexibility in job responsibilities, comply with organizational standards, and perform physical tasks including lifting, pushing, and standing for extended periods.
Qualifications
  • Completion of a Two-Year CAPTE-accredited Physical Therapist Assistant program
  • At least one year of experience
  • Proficiency in Microsoft Office Suite
Licenses and Certifications
  • Licensed Physical Therapy Assistant
  • AHA Basic Health Care Life Support HCP Certification
