Job summary

AccordCare, a premier Medicare Home Health Agency, seeks a skilled Physical Therapist Assistant to support medically prescribed therapy under a licensed PT. You will promote patient independence, build strong relationships with patients and families, and contribute to organizational success in a team-based clinical setting.

The PTA will work in patients' homes or other clinical settings, documenting care and coordinating with the supervising PT and care team to ensure quality outcomes.

Qualifications

• Current, active PTA license in the state of practice and in good standing.
• Minimum of six months of experience as a Physical Therapist Assistant in a multidisciplinary setting preferred.
• Strong communication and documentation skills.
• Demonstrated commitment to patient-centered, safe, and compassionate care.
• Ability to work effectively within a healthcare team and manage time to meet productivity standards.

Responsibilities

• Deliver skilled therapeutic treatments, including pain management and functional restoration through modalities such as heat therapy, ultrasound, and structured exercise programs.
• Monitor and document patient responses to therapy, reporting changes promptly to the supervising Physical Therapist and clinical team.
• Support the development and execution of individualized, goal-based care plans.
• Maintain timely, accurate clinical documentation in accordance with agency standards and regulatory requirements.
• Collaborate with interdisciplinary team members to coordinate patient care and participate in team meetings, in-service training, and QAPI initiatives.
• Adhere to infection control protocols, HIPAA regulations, and agency policies, ensuring patient safety and confidentiality.
• Contribute to a collaborative and professional work environment, supporting new staff orientation and demonstrating clinical accountability and teamwork.
